Below are some important steps to help you prepare for your visit:
Call 985.449.0944 to discuss your child’s needs or INQUIRE ONLINE.

CPT will help you to verify insurance benefits and discuss payment options with you.

Fax copy of insurance card to CPT at 985.449.0945.

Obtain prescription from pediatrician or attending doctor for child to be treated and evaluated at CPT. (Diagnostic codes are very helpful).

Set up Evaluation appointment

Download and Fill Out NEW PATIENT PACKET and bring with to Evaluation. In order to open file, you must call 985.449.0944 for password or email us at info@therapyforpeds.com
